The diocesan health insurance is offered through Christian Brothers Employee Benefit Trust (CBEBT) in a managed care PPO plan. The plan includes benefits for health and dental. The health and dental coverage are encompassed as one and cannot be applied for separately.
The health insurance has an annual deductible of $500 per person to a maximum of $1,500 per family, a per admission co-payment for in-network hospitals of $250 and for out-of-network hospitals of $500. The Physician Services (provided an AETNA participating physician is utilized) are paid at 100% after a $30 co-payment up to the first $300. After the $300 benefit, no calendar year deductible will apply and any remaining charge will be paid at the in-network PPO percentage (80%).
The co-payment includes lab and x-rays; preventative care including routine physicals, as well as well baby care with immunizations. Most other services are payable at 80% after the annual deductible has been met.

A Prescription Drug Program is also included in the plan. Prescriptions (retail and mail-order) are subject to a $100 calendar year deductible after which co-pays apply. The co-pay prices for 30 days of medications purchased at a retail pharmacy are: $10 for generic; $30 for preferred brand, and $50 for non-preferred brand (provided a network pharmacy is used). The plan also features a mail-order prescription service for maintenance medications. A 90-day supply of medications can be ordered at the co-pay prices of: $25 for generic; $75 for preferred brand, and $125 for non-preferred brand.
Retail purchases are limited to an initial fill and two subsequent refills for maintenance prescriptions. The member may continue to use Retail. However, the co-pay will change to the mail-order co-pay and only up to a 30-day supply can be dispensed at Retail.
Dependent Coverage is offered at the following premium rates (effective 3/1/2010) to be paid for by the employee.

Employees and dependents who choose not to participate in the health and dental plan may apply late at any time with a six-month waiting period for coverage. Any employee and/or dependent not
participating in the health and dental plan that loses other coverage may apply to the diocesan plan within 30 days of loss of other coverage with no waiting period.
All eligible employees (including those who choose not to participate in the health and dental plan) of a Diocesan institution or Catholic Charities of Southeast Texas, will be provided life, accidental death and dismemberment, and long-term disability insurance at no cost to the employee.
Term Life insurance is carried through UNUM. The death benefit for the term life is equal to 150% of your annual salary.
Accidental Death and Dismemberment insurance is carried through Mutual of Omaha. The death benefit for AD&D is 150% of your annual salary.
Long-term disability insurance for lay employees is also carried through UNUM. The benefit pays 60% of your monthly salary in coordination with any other benefits you may receive.
Long-term disability for diocesan priests incardinated in the Diocese of Beaumont is provided for in the defined benefit plan.
Long-term disability for religious personnel is the responsibility of each respective religious community.
*Worker’s Compensation Insurance is covered through an insurance carrier other than CBEBT. All job related injuries should be reported to your supervisor immediately. Medical claims for job related injuries must be filed with the Workers’ Compensation carrier NOT with CBEBT or your personal major medical insurance carrier.

Retirement
The Diocese of Beaumont offers a defined benefit plan and a defined contribution plan for retirement purposes for lay employees and priests incardinated in the Diocese of Beaumont.
Defined Benefit Plan: Contributions are made exclusively by the employer. A lay employee is eligible immediately upon hire, if he or she works at least 30 hours or more per week. Partial vesting occurs after an employee completes five years of service. An employee is 100% vested in the plan after completing 10 years of service. A priest is eligible upon his date of incardination in the Diocese of Beaumont. A priest incardinated in the Diocese of Beaumont is fully vested after 10 years of service.
Defined Contribution Plan: Eligible employees may choose to defer from 0 to 100% of his/her salary up to the Internal Revenue Service annual limit. The employee’s taxable income is reduced by the amount contributed through salary deferral. An employee becomes eligible to participate in the plan immediately upon hire, if he or she is a lay employee who works at least 30 hours or more per week or is a priest incardinated in the Diocese of Beaumont. Employees who are or will attain the age of 50 yrs during the course of the year are eligible for an additional “catch-up” contribution to the Plan as prescribed by the Internal Revenue Service. Participants are always 100% vested in the plan. The plan is managed by the Principal Financial Group.
